Immunization Program
CMSD EMPLOYEE IMMUNIZATIONS: FRIDAY, NOVEMBER 5, 2010
SIGN UP WITH YOUR SCHOOL NURSE BY 10-26-10!!
Vaccines are an integral part of adult health. While immunization programs targeted to children and adolescents are effective, CDC reports that approximately 50,000 adults die each year from vaccine-preventable diseases in the U.S (http://www.cms.gov/AdultImmunizations/).
In an effort to increase adult immunization rates and prevent the spread of disease, North MS Medical Center - Work Link has developed a School Employee Immunization Program for (interested) employees whose primary insurance carrier is BCBS of MS through our school district. (If you do not have BSBC of MS School insurance as your primary insurance carrier, you can pay $25 and get a flu vaccine (only). TheFlu, Tdap, HPV 3-shot series (for females up to 26th birthday) and the Hepatitis A&B 3-shot series will be offered and are covered 100% by the BCBS Wellness Benefit. You must complete the Web MD Health Quotient to be covered . Attached you will find the Web Md website, several forms that you will need to print, complete, sign (to bring with you), and vaccine information statements for each vaccine. You can also find information about these and other vaccines at http://www.cdc.gov/vaccines/vpd-vac/vpd-list.htm.
VACCINE SCHEDULE:
Work Link nurses will be at your school on Friday, November 5, 2010 to give the flu vaccine and the 1st Hepatitis A&B and HPV vaccines.
Approximately 5 weeks later they will come back to your school and give the Tdap vaccine and the 2nd Hepatitis A&B and HPV vaccines.
Six months from November 5, they will come back to your school to give the 3rd (last) Hepatitis A&B and HPV vaccines.
Important points to remember:
1. You must be covered under the BCBS School insurance plan
(unless you pay $25 for flu).
2. See your school nurse to sign up - deadline 10-26-10!
(Vaccines will be ordered based on the
number of employees who sign up for each one.)
3. Go on-line at least 48 hours before day of vaccines and complete
Health Risk Assessment.
4. Print, complete & sign the following forms to
bring with you on November 5:
A. Patient Information Form
(complete ONLY THE TOP PART of the form including your email address).
B. Consent form(s) for requested vaccinations
(separate form for each vaccine).
C. And, bring a COPY of the front and back of your BCBS Insurance card.
IF YOU WOULD LIKE TO RECEIVE ANY OR ALL OF THE VACCINES, YOU MUST SIGN UP WITH YOUR SCHOOL NURSE NO LATER THAN OCTOBER 26, 2010!
SHOULD ALL ADULTS BE IMMUNIZED? YES!!
Some adults incorrectly assume that the vaccines they received as a child will protect them for the rest of their lives. Generally this is true, except that: Some adults were never vaccinated as children and/or the newer vaccines were not available; immunity can begin to fade over time; and, as we age, we become more susceptible to serious diseases caused by common infections such as the flu.
Vaccination with Tdap protects adults against pertussis, but can also reduce the likelihood of transmission to infants who have not completed their primary pertussis vaccination series -- generally those less than 6 months of age, nine of whom have died during the current pertussis outbreak in California. The CDC recommends everyone ages 11 through 64 -- especially healthcare personnel and adults who have contact with or care for infants -- should get the Tdap booster vaccine to help protect themselves and reduce the risk of spreading pertussis to babies.

Spouses and Dependents
If you have a spouse or dependents 18 years and older who are covered under your school BCBS insurance (as their primary insurance carrier), they are also eligible for the vaccines. They must also have filled out the health quotient questionnaire 48 hours before Nov. 5, signed up with your school nurse by Oct. 26 and bring the forms, etc. with them on the day of the vaccines to your school location.
NMMC will ONLY accept our BCBS school insurance, so if your school insurance is considered "secondary" insurance it will not be filed and vaccines cannot be given (with the exception of the flu vaccine for $25 cash.)
